Part I: Personal Information

• Client's legal name (as titled on property), birth date, and Social Security number
• Home address, city, state/zip, email, and home phone
• Spouse's legal name, birth date, and Social Security number
• Date of marriage

Children and Family Members

List each child or family member's full legal name and birth date.

Your Concerns

Rate each item High, Some, Low, or N/A for both client and spouse:

• Providing for and protecting spouse
• Avoiding probate
• Protecting assets from lawsuits

Important Family Questions

• Are you making payments pursuant to a divorce or property settlement? (If yes, we'll ask you to upload a copy of the order.)
• Have you and your spouse signed a pre- or post-marriage contract? (If yes, we'll ask you to upload a copy.)
• Have you completed a previous will, trust, or estate plan? (If yes, we'll ask you to upload copies of the existing documents.)

Part II: Property Information

Using ownership codes — C (Client), S (Spouse), JTS (Joint w/ Spouse), JTO (Joint w/ Other) — list:

• Real property (residence, vacation home, etc.)
• Bank accounts (checking, savings, CDs)
• Retirement plans (IRAs, 401(k)s, pensions)
• Life insurance & annuities

Part III: Design Information

• Guardian for minor children (if applicable)
• Initial trustee(s) — usually client/spouse jointly
• Death trustee — who handles affairs after both spouses pass away
• Medical power of attorney (client's agent)
